Helpful Capitola Police Department forms
Housewatch form:  The information you provide will be placed in a log and addressed during the Capitola PD's daily roll call to make officers aware when you are out of town.
Shut-in/Elderly form:  The information you provide will be placed in a log and accessible to the Capitola PD officers should your loved one need help during a large scale emergency or disaster.
Wanderer/Elderly form: The information you provide will be placed in a log and accessible
to the Capitola Police Department should your loved one wander and/or be located.
